E. Declaration, Authorization & Customer's Data Privacy Consent

  1. I/We do solemnly and sincerely declare that the information given is true and correct to the best of my/our knowledge and belief.
  2. I/We understand that any false or fraudulent statements or any attempt to suppress or conceal any material facts shall render the Policy void and we shall forfeit our rights to claim under the Policy.
  3. I/We hereby authorise any hospital, Medical Practitioner or other person who has attended or examined me, to disclose when requested to do so by ChangiAssure, or its authorised representative, any and all information with respect to any illness or injury, medical history, consultations, prescriptions or treatment, and copies of all hospital or medical records. A photostat copy of this authorisation shall be considered as effective and valid as the original.
  4. I/We hereby authorise and request ChangiAssure to pay benefit due in respect of this claim to :

Important Notice (Claims)

ChangiAssure policies are underwritten by HL Assurance Pte. Ltd. (“HLAS”). Changi Travel Services Pte Ltd (“CTS”) has been authorized by HLA to process claims pertaining to ChangiAssure policies (“Claims”) at the Changi Recommends counters located at Singapore Changi Airport. CTS is only authorized to process the specific types of Claims and the maximum amount under such Claims as set out in the table below:

Please note that:

  1. Claims are subject to all the exclusions set out in the Policy Document.
  2. Claims which do not come within the types of Claims and/or which exceed the maximum amount set out in the table above shall not be assessed at the Changi Recommends counters at Changi Airport.
  3. Acceptance of the claim form and any purported supporting documents does not mean admission of legal responsibility for your Claim.
  4. Failure to provide the relevant supporting documents or the provision of incomplete supporting documents may lead to delay in the processing of your Claim and/or your Claim being rejected.
  5. CTS has been authorized by HLAS to conduct the assessment of your Claim but HLAS has full discretion to make the final decision on all Claims.
